What we collect
Information you give us during intake (health history, medications, goals, contact details), information generated during care (visit notes, prescriptions, lab results), and basic technical data such as device type and pages visited.
How we use it
To provide and coordinate your medical care, to verify eligibility and licensure in your state, to bill your membership, and to meet legal and professional recordkeeping obligations. We do not sell your information, and we do not share it with advertisers, data brokers, or employers.
Who can see it
Your treating physician and the clinical staff supporting your care. With your written permission, your primary care physician or another clinician you name. Our technology vendors, under business associate agreements that bind them to HIPAA. Law enforcement or public health authorities only where the law requires it.
Protected health information and HIPAA
All Good Health Clinic is a covered entity under the Health Insurance Portability and Accountability Act. Your protected health information is stored encrypted at rest and in transit, access is limited to staff with a treatment relationship to you, and access is logged. You may request our full Notice of Privacy Practices at any time.
The AI care navigator
The care navigator on our site sends the text you type to an AI model to generate general program information. Do not enter your name, date of birth or identifiers there. Navigator conversations are not part of your medical record and are not reviewed by a clinician unless you tell us to.
Your rights
You may request a copy of your record, ask us to correct it, ask for a list of disclosures, restrict certain sharing, or ask us to delete information we are not required to retain. Email privacy@allgoodhealthclinic.com and we will respond within 30 days.
Retention
Medical records are retained for the period required by the law of the state in which you were treated, typically seven to ten years after your last visit. Marketing and website analytics data is retained for 24 months.
